To Order - Complete the order form below and pay for your order.
Payments can be made via cash, check, or Venmo. Checks can be made payable to Desert Sage PTO. Send check or cash with the first and last name used to place your order in an envelope marked PTO. Envelopes can be given to your child's teacher or dropped off at the front office. To pay with Venmo, follow the link at the end of the order form and label your payment "shirts". Be sure to come back to this tab to finish submitting your order.
